Background
Amref Health Africa is the largest Africa-based international health development organization, serving over 30 million people annually across 35 countries in sub-Saharan Africa. For 65 years, we have partnered with communities, governments, and local partners through our European and North American network on program implementation, advocacy, fundraising, and partnerships. Our subsidiaries include Amref Flying Doctors, Amref Enterprises Limited, and Amref International University.
In Ethiopia, Amref has been active since the 1960s and became fully operational in 2002. For over twenty years, we have collaborated with the Ethiopian government and local communities to implement diverse health and development projects throughout the country. Our work aligns with the national Medium-Term Health Sector Investment and Development Plan (HSDIP), focusing on health equity for women and children, reaching disadvantaged communities including pastoralist populations, and emphasizing youth development and health system strengthening.
Amref Health Africa in Ethiopia has developed an eight-year strategic roadmap (2023-2030) to realign the organization with current opportunities and challenges. While strategic indicators, targets, and baseline assessments are in place, a comprehensive costing of the strategy implementation is needed. Therefore, Amref seeks to engage a consultant to conduct a detailed costing of its strategic roadmap and perform a resource gap analysis.
Objectives of the Assignment
The primary objective is to develop comprehensive cost estimates for implementing Amref Health Africa in Ethiopia’s strategic roadmap (2025-2030). This assignment has two primary objectives:
1. Estimate Implementation Costs: The consultant will develop detailed cost estimates for implementing the strategic roadmap using activity-based costing methodology. This will include comprehensive financial projections for achieving targeted organizational KPIs, broken down by program areas, geographical locations, and implementation timelines.
2. Resource Gap Analysis: A thorough analysis of existing resource envelopes will be conducted for all current projects and programs. This analysis will identify funding gaps and opportunities, enabling informed decision-making for resource mobilization efforts. The analysis should be conducted at the activity level to ensure a detailed understanding of resource needs.
Scope of Work
Once awarded, the consultant will undertake the following activities:
1. Review Strategy Costing Approaches:
The consultant will conduct a comprehensive review of existing strategy costing approaches currently used by NGOs in the health sector. This review should examine best practices from similar organizations, particularly those operating in Africa. The analysis should include an evaluation of different costing methodologies, their advantages and limitations, and their applicability to Amref’s context. This review will form the foundation for developing appropriate costing approaches for Amref’s strategic roadmap.
2. Review Amref Ethiopia’s Strategic Roadmap:
A thorough understanding of Amref’s strategic roadmap is required, with a particular focus on strategic indicators (KPIs) and their associated targets. Understanding the Strategic Roadmap will form the basis for developing an appropriate costing methodology and tools for data abstraction and analysis.
3. Develop Costing Methods and tools:
The consultant will create a comprehensive costing methodology that ensure clear linkages between program activities and organizational KPIs. This methodology must establish clear hierarchical relationships between activities and outcomes. The methodology should enable Amref to track how individual program activities contribute to broader organizational goals. As the strategy KPIs are expected to be linked to the various projects and programs implemented by Amref Ethiopia, the consultant is expected to come up with a clear mapping methodology to map the relationship and contribution of activities to the strategy KPIs.
4. Design and Implement Costing Framework:
Using activity-based costing methodology, the consultant will develop a detailed costing framework that captures all aspects of implementing the strategy roadmap. This framework must:
• Generate cost estimates categorized by project, geographic location, and target population
• Take into account the estimation of shared costs across multiple programs
5. Conduct Comprehensive Cost Analysis:
The analysis phase will involve several key components:
• Historical Data Review: Examine past expenditure on different activities within Amref’s past and existing projects/programs
• Current Project Analysis: examine ongoing projects’ alignment with strategic objectives and their resource utilization
• Stakeholder Consultation: Gather input from key stakeholders through structured interviews and workshops
• Provide costs of different activities under each project/program
• Account for both financial and economic costs associated with strategy implementation
• Account for both overall and incremental costs associated with strategy implementation
• Based on the existing resource envelope, conduct the resource gap analysis and provide recommendations for resource mobilisation
• All costs should be estimated for the period: 2025 – 2030
• Scenario Planning: Develop multiple scenarios considering various internal and external factors
• Sensitivity Analysis: Assess the impact of key variables on overall strategy costs
6. Facilitate Validation Process:
The consultant will lead three structured validation workshops:
- Workshop 1 – KPI-Project Activity Mapping:
Present and validate the methodology for linking projects to organizational KPIs. This session will ensure alignment between operational activities and strategic objectives. - Workshop 2 – Costing Approach:
Review the proposed costing methodology and preliminary cost estimates with key stakeholders. This workshop will validate assumptions and calculation methods. - Workshop 3 – Final Review:
Present the complete analysis, including cost projections, resource gap analysis, and recommendations for implementation. This session will secure final stakeholder approval and commitment.
Deliverables
Inception Report: The consultant will produce a comprehensive inception report that includes:
- Detailed work plan with specific timelines and milestones
- Complete methodology for both costing and resource gap analysis phases
- Excel File with Mapping of all projects to organizational KPIs and related activities
- Report outlines for all proposed deliverables
Project/program contribution to strategy KPI indicators report: a report showing the mapping exercise done between projects and their contribution to the different organizational KPIs. It should include a summary Word document and a detailed Excel file.
Data Collection Tools: A comprehensive suite of tools must be developed including:
- Standardized data collection templates
- Interview and focus group discussion guides
- Cost capture spreadsheets for costing exercise
- Activity and indicator mapping tools
- Data validation protocols
Strategy Costing Analysis: A detailed costing analysis including:
- Activity-based cost breakdowns for all strategic initiatives
- Unit costs for key interventions
- Cost projections under different scenarios
- Sensitivity analyses of key cost drivers
- Validation of the preliminary cost estimates and resource gap analysis
Strategy costing draft report: a report showing the results of the costing exercise for the strategy. It should clearly show the funding available and the gaps. The report should have an excel annex file with detailed calculations.
Final Report and Presentation: The final deliverables must include:
- Comprehensive written report with executive summary
- Detailed Cost Estimates in an Excel file showing the breakdown of the costs
- PowerPoint presentation for stakeholders
- Implementation recommendations
- Monitoring framework for tracking costs against projections
Final report and presentation: A comprehensive report detailing the strategy financial projection, strategy KPI funding requirements and resource gaps. A final presentation summarizing key findings and recommendations, presenting these to senior management and stakeholders
Duration of the Assignment
The consultancy will span 4 months, with specific phases allocated as follows:
- Inception and planning: 2 weeks
- Data collection and analysis: 8 weeks
- Initial reporting and validation: 3 weeks
- Refinement and final reporting: 3 weeks
About You
Qualifications of the Consultant
The successful consultant or consulting firm must demonstrate:
Professional Experience:
- Minimum 10 years’ experience in strategic costing and financial modelling
- Demonstrated expertise in activity-based costing methodologies
- Experienced in various standard costing models and tools including indicator and activity-based costing
- Expertise in undertaking resource gap analyses
- Excellent communication skills, particularly in presenting complex costing data in accessible terms
Technical Skills:
- Advanced financial modeling capabilities
- Strong data analysis and visualization skills
- Expertise in scenario planning and sensitivity analysis

Payment Terms
Payments will be structured in three phases:
Phase I (20%):
- Upon submission and approval of the inception report
- Completion of all data collection tools
- Initial work plan approval
Phase II (30%):
- Completion of preliminary analysis
- Submission of draft reports
- Validation workshop delivery
Phase III (50%):
- Final report submission
- Presentation delivery
- All deliverables approved
- Documentation completed
Additional expenses (workshops, travel) will be covered directly by Amref.
Confidentiality and Compliance
The consultant must adhere to strict confidentiality requirements including:

The consultant shall treat all information obtained during this assignment as strictly confidential. This includes but is not limited to, any data, documents, financial information, stakeholder insights, and internal discussions shared by Amref or accessed by the consultant in the course of the consultancy. The consultant agrees to:
- Not disclose any confidential information to third parties without prior written consent from the organization.
- Use the information solely for the agreed-upon assignment and not for any other purpose.
- Implement necessary measures to protect confidential information from unauthorized access, loss, or misuse.
Upon completion or termination of the consultancy, the consultant shall return or destroy all confidential materials as instructed by Amref. Any reports or deliverables produced under this consultancy will become the property of Amref Health Africa in Ethiopia, and Amref retains full rights over their distribution, publication, and use.
The consultant is expected to comply with Amref Health Africa’s compliance-related laws including the organization’s code of conduct ethical guidelines, data protection laws, intellectual property rights and others. Failure to adhere to the above confidentiality and compliance standards may result in immediate termination of the contract and may subject the consultant to further legal action.
Reporting and Supervision
The consultant will work under the following reporting structure:
